Can a sole beneficiary of real property be transferred by myself (the benificary) without the nedd for an attorney

I am 62 years old and would like to put the title in my childrens name in case I am in a position to require long term care but keep the existing mortgage in my name. Also can the existing mortgage be transferred to my name without having to apply for a new mortgage.
