What form do i use to file a law suit in circuit court of douglas county colorado

My house was broken into several months ago. My claim was approximately $50,000. Farmers insurance company told me my claim was to high and said they would not pay the claim. I have a police report and also they sent out a crime lab for finger prints but farmers insurance co. said they will still not pay the claim

You can file a civil complaint, you would file the complaint along with a civil cover sheet. i would recommend getting attorney to represent you on the matter, an attorney like myself often take insurance dispute cases like this on contingency fee basis, meaning we don't get paid unless you do.
